Administered through Indiana's Medicaid HCBS (Home and Community-Based Services) Waivers, it allows family members to receive a flat weekly stipend based on the care recipient's assessed level of need.

Unlike programs that pay by the hour, Indiana SFC pays a fixed weekly rate regardless of how many hours you provide — making it predictable and sustainable. The level (1, 2, 3, or ATTC) is determined by a care manager from your assigned Managed Care Entity (MCE) during an in-home assessment.

Key fact: Indiana SFC does NOT require the caregiver to be a certified HHA or CNA. The training is provided by the SFC agency (like Paid.care) and takes about 6–8 hours. This makes Indiana SFC one of the fastest programs to enroll in — most families receive their first paycheck within 30–60 days of application approval.

What the SFC Assessment Evaluates

Your MCE care manager will evaluate how many Activities of Daily Living (ADLs) your loved one needs help with: bathing, dressing, eating, mobility, toileting, and medication management. The person receiving care must meet these criteria to qualify for Structured Family Caregiving through Medicaid:

What does an SFC level-of-care assessment involve?
The level-of-care assessment is conducted by a care manager from your assigned Managed Care Entity (MCE) — either Humana, Anthem, or UnitedHealthcare in Indiana. The care manager visits your home and evaluates: which ADLs the care recipient needs help with (bathing, dressing, eating, mobility, toileting, medication management), their cognitive status and supervision needs, any extraordinary medical needs (feeding tubes, wounds, oxygen, etc.), and their overall functional status. Can I work another job while receiving SFC pay?
Yes — receiving SFC pay does not prohibit you from holding other employment. Many SFC caregivers work part-time or continue some form of employment. However, you must be able to fulfill your SFC care obligations (the care hours documented in your care plan) while maintaining other employment. What training does an SFC caregiver need?
SFC caregiver training is significantly less intensive than HHA or CNA certification. The MCE care manager will conduct a new assessment, and if the new level is higher, your weekly pay increases accordingly. Does SFC affect my loved one's Medicaid benefits?
No — SFC compensation goes to the caregiver (you), not the care recipient. Can I get SFC pay if my family member has both Medicare and Medicaid?
Yes — being dual-eligible (both Medicare and Medicaid) actually opens additional doors. The care recipient still qualifies for SFC through their Medicaid HCBS Waiver. Additionally, if they have dementia, the Medicare GUIDE Program (launched July 2025) may provide additional caregiver support and respite pay for a second family member simultaneously.
